Introduction

Pathogenesis

Etiology

  • Direct impact   →   tibial malleoli, trochlear ridges, calcaneal tuber and fourth tarsal bone (the latter usually in combination with injury to fourth metatarsal bone   MC / MT 2 and 4: fracture  ).
  • Trapped limb, eg in cattle grid or fence   →   luxations and fractures of the body of the talus, other major tarsal fractures.

Pathophysiology

  • Strong collateral and periarticular ligaments plus insertions of cranial tibial and peroneus tertius muscles provide marked stability to the joint.
  • Direct blows to the hock can result in an open fracture and   →   osteomyelitis, deep digital flexor tenosynovitis   Tarsus: tarsal sheath tenosynovitis  and luxation of the superficial digital flexor tendon   SDFT: luxation  .
